We are currently seeking a dedicated and experienced Qualified Social Worker (QSW) to join our team at a reputable hospital in Portsmouth. As a QSW in this role, you will play a crucial part in providing support and assistance to patients and their families during their hospital stay.

Key responsibilities and duties:

  • Conduct comprehensive assessments of patients' social care needs
  • Develop and implement care plans in collaboration with the multidisciplinary team
  • Provide emotional support and guidance to patients and their families

Essential qualifications and skills:
