We are currently seeking a dedicated and experienced Assessment Social Worker to join our team in Stockton on Tees. As an Assessment Social Worker, you will play a crucial role in conducting assessments to determine the needs and risks of vulnerable individuals and families, and develop appropriate support plans to safeguard their well-being.

Key responsibilities and duties include:

  • Conducting thorough assessments of service users' needs and risks
  • Developing and implementing care plans in collaboration with service users, families, and other professionals
  • Providing support and guidance to service users to promote independence and well-being
  • Participating in multi-agency meetings and reviews to ensure coordinated care
  • Keeping accurate and up-to-date records of assessments and interventions

The essential qualifications and skills for this role include:

  • Qualified Social Worker with current registration with Social Work England
  • Experience in conducting assessments and developing care plans within a social care setting
  • Strong communication and interpersonal skills to engage effectively with service users and professionals
  • Ability to work autonomously and as part of a multidisciplinary team
  • Knowledge of relevant legislation and policies in social work practice
