We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Team in Middlesbrough. Your key responsibilities will include:
- Conducting assessments to identify the needs of adults and developing tailored support plans.
- Providing ongoing support and guidance to clients and their families, ensuring they have access to necessary resources and services.
- Working closely with other professionals and agencies to coordinate care and support for vulnerable adults.
- Maintaining accurate and up-to-date records of casework and interventions in compliance with organizational policies.
- Participating in team meetings and supervision to enhance practice and share best practices.
To be considered for this role, you must possess the following qualifications and skills:
- A recognized social work qualification (Degree in Social Work, DipSW, or equivalent).
- Registration with Social Work England.
- Demonstrable experience in adult social work, preferably in a community setting.
- Strong assessment and analytical skills, with the ability to develop effective care plans.
- Excellent communication and interpersonal skills, with a commitment to working in partnership with clients and stakeholders.
