We are looking for an experienced Adults Advanced Practitioner to join the Working Age Community Mental Health Team (CMHT) in Warrington. This role is ideal for a skilled and motivated social worker who thrives in a collaborative, multidisciplinary setting and is committed to delivering high-quality care and support.

Key Responsibilities:
Complete Care Act assessments

Arrange and monitor care packages to ensure their ongoing effectiveness

Monitor mental health and support individuals in maintaining their well-being

Work as part of a multi-disciplinary team (MDT) alongside health professionals to manage risk

Record accurate and up-to-date case notes on the Paris system

Manage safeguarding responsibilities and purchase care packages using Mosaic

Requirements:
Social Work qualification (CQSW, DipSW, CSS, or equivalent)

Registered with Social Work England

Substantial post-qualifying experience in adult mental health

Familiarity with Paris and Mosaic recording systems

Strong understanding of Care Act legislation, safeguarding, and risk management

Ability to work effectively both independently and within a team
