Social worker - Adults care team - South Glos
Social Worker required for work within the Adult social care Team in the South Gloucestershire area. Rates are £32.65ph. 6 month initial contract. Exciting opportunity in a friendly and supportive team. Based at the Badminton Road Offices. Hybrid working between office and home.
Eden Brown Synergy is seeking an experienced and dedicated Qualified Social Worker to join the Adult Social Care team in South Gloucestershire. This is an essential role where you will play a vital part in assessing needs, providing advice and guidance, and ensuring high-quality care and support for our service users.
Key Responsibilities:
Conduct social work assessments to identify the needs of service users and provide appropriate advice and guidance.
Liaise closely with contacts from health, voluntary, and other sectors, working as part of a multi-disciplinary team to share best practices and ensure effective communication.
Review and monitor care plans, responding to changes in service users needs.
Develop and evaluate rabblement programs within care packages to support independence.
Manage your own caseloads or specific projects, ensuring specialist inputs are obtained and effective outcomes are achieved.
Raise awareness of care issues, conduct risk assessments, and develop safe working practices to meet assessed needs.
You will be Social Work England registered, ideally a driver with access to own car and able to reside and work in the UK
