We are currently seeking an experienced Qualified Social Worker to join our All Age Disability Team in Wrexham. This role offers the opportunity to work within a well-established, supportive, and collaborative environment. The team is known for its strong culture of peer support and regular, reflective supervision provided by experienced Assistant Team Managers and the Team Manager.

Key Responsibilities:

Undertake statutory social work with adults with diagnosed disabilities in accordance with the Social Services and Wellbeing (Wales) Act 2014

Complete strength-based assessments and care planning

Work in partnership with individuals, families, and multi-agency professionals

Maintain accurate and timely case records in line with service expectations

What We're Looking For:

Qualified Social Worker registered with Social Care Wales

Significant experience in adult social care, preferably disability services

Familiarity with the SSWB (Wales) Act 2014

Full driving licence and access to a vehicle (mileage paid at £0.45p per mile)

Strong team ethic and commitment to person-centred practice

Working Arrangements:

Hybrid working in line with Wrexham Council's flexible policy

Office base at Crown Buildings

Mileage paid for home visits and fieldwork
