Wirral Council is seeking an experienced and dedicated Qualified Social Worker to join the Adults Community Team, based in Pennant House, Bebington.

In this role, you will be responsible for delivering high-quality, person-centred care and support to adults within the community, in line with the Care Act 2014. You will work closely with individuals, their families, and partner agencies to complete assessments, develop care plans, and carry out reviews that promote independence and well-being.

Key Responsibilities:

Undertake Care Act assessments and reviews

Conduct Mental Capacity Assessments

Lead and contribute to safeguarding investigations

Develop person-centred support plans

Work in partnership with health professionals, providers, and community organisations

Maintain accurate and up-to-date case records

Work autonomously while managing a varied caseload

Requirements:

Social Work qualification (Degree/DipSW/CQSW)

Registration with Social Work England

Proven post-qualifying experience in Adults Community Social Work

Strong knowledge of Care Act, Mental Capacity Act, and safeguarding legislation

Ability to work independently and within a multi-agency team
