Role 1: Qualified Social Worker - West Locality Team

Join the West Locality Team and work in a supportive, established service focused on delivering high-quality, person-centred care.

Key Responsibilities

  • Undertake strengths-based assessments under the Care Act 2014
  • Deliver trauma-informed and person-centred practice
  • Support adults with a wide range of needs including:
    • Autism
    • Physical disabilities
    • Learning difficulties
    • Acquired brain injuries
    • Addiction and complex trauma
  • Develop outcome-focused care and support plans
  • Promote independence and wellbeing

Working Environment

  • Hybrid working (home and Fountain Court office)
  • Collaborative, experienced team
  • Close partnership working with:
    • South Tees Hospitals NHS Foundation Trust
    • Tees Esk and Wear Valleys NHS Foundation Trust
    • Accessing Change Together (ACT) Middlesbrough

Role 2: Qualified Social Worker - Adult Social Care

This role offers the opportunity to contribute to a broad Adult Social Care service, supporting residents across Middlesbrough.

Key Responsibilities

  • Complete comprehensive, person-centred assessments
  • Identify needs while considering the individual's circumstances and voice
  • Develop care plans using:
    • Community and informal support networks
    • Statutory services where required
  • Safeguard adults from abuse and neglect
  • Work collaboratively within multi-disciplinary teams

  • Be able to manage a busy caseload and meet deadlines
  • Be committed to multi-agency working
  • Be proactive and innovative in practice
  • Be able to meet travel requirements
  • Be registered (or eligible) with Social Work England
