We are seeking a dedicated and experienced Qualified Social Worker to join the West Northamptonshire ATM Learning Disability Team on a contract basis, offering an attractive rate of £38 per hour. This role is pivotal in supporting individuals with learning disabilities, ensuring they receive the necessary care and guidance to lead fulfilling lives within their communities.

As a member of our team, you will take on key responsibilities including:

  • Conducting comprehensive assessments of individuals with learning disabilities to identify their needs and support requirements.
  • Developing and implementing personalized care plans in collaboration with clients, families, and other professionals.
  • Providing ongoing support and guidance to individuals and their families, ensuring they are informed about available resources and services.
  • Working closely with multidisciplinary teams to ensure integrated service delivery and effective communication.
  • Maintaining accurate and up-to-date case records and documentation in accordance with legal and organizational standards.

The ideal candidate will possess essential qualifications and skills, including:

  • A recognized social work qualification (e.g., BA/MA in Social Work or equivalent).
  • Registration with Social Work England.
