Hours: 21 hours per week (flexible spread over 3-4 days)

Pay Rate: £30 per hour

About the Role:

Eden Brown Synergy is currently recruiting for an Experienced Adult Social Worker to join the Preparation for Adulthood Team within Adult Future Options in Sheffield. This is an exciting opportunity to support young people aged 16 and above who are transitioning from children's services to adult services.

The team plays a crucial role in the transition process and works with complex young people to ensure their needs are met as they move into adulthood. As a Qualified Social Worker, you will carry out Care Act Assessments, support Mental Capacity and Best Interest Assessments, and assist with Court of Protection applications. This post also includes the completion of the first Care Act assessment for each young person in line with statutory requirements.

Key Responsibilities:

  • Care Act Assessments: Complete initial Care Act assessments for young people transitioning from children's services.
  • Mental Capacity & Best Interest Assessments: Support the planning and implementation of mental capacity assessments and best interest decisions.
  • Court of Protection Applications: Prepare and submit applications as required.
  • Support Transitioning Young People: Work closely with young people, their families, and relevant agencies to ensure smooth transitions into adult services.

Essential Requirements:

  • Qualified Social Worker with valid registration (SWE)
  • Experience working with young people and adult services, particularly with transitions from children's to adult services.
  • Knowledge of Care Act 2014, Mental Capacity Act 2005, and related statutory frameworks.
  • Strong communication and assessment skills.
  • Ability to work independently and as part of a multi-disciplinary team.
