Westminster - QSW - Mental Health Team - £32 p/h Umb
Eden Brown Synergy are currently looking for an experienced Mental Health Social Worker to join the Adult Social Care Mental Health Team for Westminster City Council
Duties and Responsibilities:
* Acting as the named/link Social Worker for a Recovery House service, supporting residents with recovery-focused interventions.
* Undertaking strengths-based Care Act assessments, care planning, reviews, and reassessments.
* Leading on discharge planning from acute and psychiatric hospital settings.
* Completing safeguarding enquiries as Lead Enquiry Officer (LEO).
* Undertaking Mental Capacity Act assessments and best interest decisions.
* Developing and reviewing Section 117 aftercare plans in partnership with health colleagues.
* Working within a duty rota, responding to referrals and enquiries in a timely manner.
* Attending MDT meetings, ward rounds, and contributing to collaborative care planning.
* Working closely with health, housing, and community partners to deliver effective outcomes.
Working Pattern:
* Hybrid working (subject to agreement)
Pay Rate:
* £32 p/h Umb
Essential Requirements:
* Social Work Qualification - Degree or equivalent
* Social Work England Registration
* Experience within a Community Mental Health Team
* Strong knowledge of the Care Act 2014, Mental Capacity Act, and safeguarding legislation
