Hackney - QSW - Mental Health Team - £32 p/h Umb
Eden Brown Synergy are currently looking for an experienced Qualified Social Worker to join the Mental Health Team for Hackney Council.
Duties and Responsibilities:
* Manage a caseload of adults with mental health needs, undertaking assessments, reviews, and support planning.
* Complete Care Act assessments, Mental Capacity Act assessments, safeguarding enquiries, and best interest decisions where required.
* Work closely with multidisciplinary teams, including NHS Mental Health Services, to deliver integrated and person-centred care.
* Promote independence and recovery through strengths-based interventions and effective care planning.
* Maintain accurate and timely case records in accordance with statutory requirements and council policies.
* Liaise with service users, families, carers, and partner agencies to ensure positive outcomes.
* Ensure all work is completed in accordance with relevant legislation, policies, and best practice guidance.
Working Pattern:
* Hybrid working.
Pay Rate:
* £32 p/h Umbrella.
Essential Requirements:
* Social Work Qualification - Degree or equivalent.
* Social Work England Registration.
* Significant post-qualified experience within Adult Social Care.
* Previous experience working within a Mental Health Team.
* Good working knowledge of the Care Act 2014, Mental Capacity Act 2005, Mental Health Act 1983, and safeguarding legislation.
