Westminster - QSW - Complex Care Team - £32 p/h Umb
Eden Brown Synergy are currently looking for an experienced Qualified Social Worker to join the Complex Care Team for Westminster City Council.
Duties and Responsibilities:
* Managing a complex caseload within the Complex Care Team.
* Undertaking Care Act assessments, support planning, reviews, and risk assessments.
* Supporting adults with complex needs including physical disabilities, mental health, learning disabilities, and safeguarding concerns.
* Carrying out Mental Capacity Assessments and Best Interest decisions in accordance with the Mental Capacity Act.
* Working in partnership with health professionals, housing, providers, and other multi-agency partners to deliver effective care and support.
* Managing safeguarding enquiries and ensuring appropriate interventions are implemented.
* Maintaining accurate case notes, assessments, and reports in line with statutory legislation and council procedures.
* Promoting independence, wellbeing, and person-centred practice at all times.
Working Pattern:
* Hybrid working
Pay Rate:
* £32 p/h Umb
Essential Requirements:
* Social Work Qualification - Degree or equivalent
* Social Work England Registration
* Previous experience working within Adult Social Care/Complex Care services
* Sound knowledge of the Care Act 2014, Mental Capacity Act, and safeguarding legislation
* Ability to manage complex caseloads within a fast-paced environment
