Eden Brown Synergy is currently recruiting experienced Occupational Therapists to join the London Borough of Hackney Adult Social Care Service. We have opportunities across Community Reviews, Sensory Services and Reablement, working within Hackney's innovative neighbourhood model to deliver strengths-based, person-centred interventions that maximise independence and improve outcomes for local residents.

These roles offer the opportunity to work within integrated multidisciplinary teams, supporting adults with complex physical disabilities, sensory impairments and long-term conditions to remain independent within their own homes while preventing, reducing or delaying the need for ongoing care and support.

Available Teams

Community Reviews

As part of the Community Review Team, you will undertake comprehensive reviews of existing care packages, equipment and adaptations, ensuring services remain appropriate, proportionate and focused on promoting independence.

You will work closely with residents, carers and partner agencies to identify opportunities for prevention, reduce long-term dependency on services and support people to achieve the best possible outcomes through Occupational Therapy interventions.

Sensory Service

Working within Hackney's specialist OT and Sensory Service, you will complete holistic assessments for adults with complex physical disabilities and sensory impairments, recommending specialist equipment, assistive technology, Telecare and major adaptations that enable residents to live safely and independently.

The role places a strong emphasis on early intervention, prevention and personalised support, helping residents maintain independence and avoid unnecessary escalation of care needs wherever possible.

Reablement Team

Hackney's innovative Reablement at Home Service supports adults following hospital discharge or community referral through short-term, therapy-led interventions.

Working alongside Reablement providers, you will develop personalised therapy plans, monitor progress and review outcomes over a period of up to six weeks, enabling residents to regain confidence, rebuild daily living skills and reduce reliance on long-term care services.

This service adopts a "doing with, not doing for" approach, empowering individuals to maximise independence while supporting safe hospital discharge and preventing avoidable admissions and ongoing care packages.

Key Responsibilities

  • Carry out comprehensive Occupational Therapy assessments within the community.
  • Complete assessments under the Care Act 2014, Mental Capacity Act, Housing Grants, Construction and Regeneration Act and other relevant legislation.
  • Undertake functional assessments to support care planning and promote independence.
  • Review existing care packages, identifying opportunities to reduce, prevent or delay long-term support where appropriate.
  • Assess, prescribe and review specialist equipment, Telecare and assistive technology.
  • Recommend and specify complex and major adaptations.
  • Deliver strengths-based, person-centred interventions focused on achieving meaningful outcomes.
  • Identify, assess and manage risk within the home environment.
  • Work collaboratively with social workers, health professionals, housing teams and voluntary organisations.
  • Contribute to safeguarding enquiries and multi-agency working.
  • Maintain accurate case recording and professional documentation.
  • Supervise Occupational Therapy Assistants and students where required.
  • Apply sound clinical reasoning and evidence-based practice in all decision making.

About You

To be successful you will have:

  • Degree in Occupational Therapy.
  • HCPC Registration.
  • Experience working within Adult Social Care.
  • Strong knowledge of:
    • Care Act 2014
    • Mental Capacity Act
    • Housing Grants, Construction and Regeneration Act
    • Safeguarding Adults
  • Experience completing community-based functional assessments.
  • Knowledge of equipment, moving & handling, adaptations and Telecare.
  • Experience working within multidisciplinary teams.
  • Excellent assessment, communication and report writing skills.
  • A strengths-based approach focused on prevention, enablement and promoting independence.
