We are currently seeking a dedicated and experienced Adult Qualified Social Worker (QSW) for a Discharge To Assess (Hybrid) position in Bristol. As a key member of our team, you will play a crucial role in supporting adults in their transition from hospital to community settings.

Key responsibilities and duties:

  • Conducting assessments to determine the needs of individuals being discharged from hospital
  • Developing and implementing care plans to support clients in their recovery and rehabilitation
  • Working collaboratively with healthcare professionals, service users, and their families to ensure a holistic approach to care
  • Providing advocacy and support to clients to access appropriate services and resources
  • Monitoring and reviewing the progress of clients to ensure their ongoing well-being and independence

Essential qualifications and skills:

  • Qualified Social Worker with relevant experience in adult social care
  • Registered with Social Work England
  • Strong assessment and care planning skills
  • Excellent communication and interpersonal skills
  • Ability to work effectively within a multidisciplinary team
