Adult's SW - Hospital Discharge Team - Halton
We are currently seeking a dedicated and experienced Adult's Social Worker to join our Hospital Discharge Team in Halton. As a vital member of the team, you will be responsible for facilitating safe and timely discharges for adult patients, ensuring they receive appropriate support and services to transition back to their homes or community settings.
Key responsibilities and duties:
- Conduct comprehensive assessments of patients' needs and develop personalized discharge plans.
- Liaise with healthcare professionals, families, and external agencies to coordinate post-discharge support services.
- Advocate for patients' rights and ensure their voices are heard in the discharge planning process.
- Provide emotional support and guidance to patients and their families during the transition period.
Essential qualifications and skills:
- Qualified Social Worker with current registration with the HCPC.
