Adult's SW - Assessment Team - Bournemouth
We are currently seeking a dedicated and experienced Adult's Social Worker to join our Assessment Team in Bournemouth. As a key member of our team, you will be responsible for conducting comprehensive assessments of adults in need of support and services, as well as developing and implementing care plans to address their individual needs.
Key responsibilities and duties include:
- Conducting thorough assessments to identify the needs and risks of vulnerable adults
- Working collaboratively with service users, families, and other professionals to develop and implement effective care plans
- Providing support and guidance to service users to promote their independence and well-being
- Maintaining accurate and up-to-date case records and reports
- Participating in multi-disciplinary meetings and reviews to ensure the best outcomes for service users
The ideal candidate for this position will possess the following essential qualifications and skills:
- Qualified Social Worker with a valid HCPC registration
- Proven experience working in adult social care, specifically within assessment teams
- Strong assessment and analytical skills with the ability to make evidence-based decisions
- Excellent communication and interpersonal skills to engage effectively with service users and stakeholders
- Ability to work autonomously and as part of a team in a fast-paced environment
