We are currently seeking a Qualified Social Worker to join our Assessment team in Hertfordshire (Hemel Hempstead) with a focus on supporting children and young people aged 0-25 with complex needs. This is a hybrid position offering a pay rate of £37 per hour.
Key responsibilities and duties include:
- Conducting assessments to determine the needs of children and young people with disabilities and complex health conditions.
- Developing and implementing care plans to support the well-being and development of individuals within the 0-25 age range.
- Working collaboratively with families, carers, and other professionals to ensure a holistic approach to care and support.
- Participating in multi-agency meetings and reviews to monitor progress and make necessary adjustments to care plans.
Essential qualifications and skills:
- Qualified Social Worker with current registration with Social Work England.
- Demonstrated experience working with children and young people with disabilities and complex needs.
- Strong assessment and analytical skills to identify individual needs and develop tailored care plans.
- Excellent communication and interpersonal skills to engage with diverse stakeholders and build effective relationships.
