Southwark - QSW - Transfer of Care Team
We are currently seeking a dedicated and experienced Qualified Social Worker to join our Southwark Transfer of Care Team. As a key member of the team, you will be responsible for facilitating safe and timely discharges for service users from hospital settings to their preferred place of care.
Key responsibilities and duties:
- Conducting assessments and developing care plans in collaboration with service users, their families, and multi-disciplinary teams.
- Ensuring that all discharges are carried out in accordance with relevant legislation and best practice guidelines.
- Providing support and guidance to service users to enable them to transition smoothly from hospital to community-based care.
- Working closely with healthcare professionals, social care teams, and external agencies to coordinate services and resources for service users.
Essential qualifications and skills:
- Qualified Social Worker with current registration with Social Work England.
- Demonstrated experience in working within adult social care, preferably in a hospital discharge or transfer of care setting.
- Strong assessment and care planning skills, with the ability to manage complex cases effectively.
- Excellent communication and interpersonal skills, with the ability to build rapport with diverse service users and stakeholders.
