Social Worker - Community Neuro Rehab Team (CNRT & NETT)
Rate: £33.76 per hour
Location: Southwark Council, London
Contract: Locum

Southwark Council is seeking an experienced Social Worker to join the Community Neuro Rehab Team (CNRT) and Neuro-Rehab Enhanced Transition Team (NETT). You will work alongside a multidisciplinary team to support patients with neurological conditions, contributing to effective hospital discharge, short-term interventions, and long-term recovery support.
Key Duties:
Attend in-person MDTs and online huddles.
Deliver Care Act assessments, safeguarding, MCA, and referrals.
Facilitate complex transitions from hospital/rehab into the community.
Provide short-term intensive support (up to 12 weeks).
Support patients over 18 with long-term neurological conditions.
Requirements:
Proven experience in adult social care and neurorehabilitation.
Strong MDT collaboration and communication skills.
Ability to work autonomously and manage caseloads effectively.
