? Pay Rate: Up to £30 per hour
? Position: Full-Time | Hospital-Based
Southwark Council is seeking an experienced Qualified Social Worker (QSW) to join the Transfer of Care Team (TOC South) based at Kings College Hospital. This is a fantastic opportunity to work in a high-performing team, providing vital services to older adults and people with physical disabilities.
? Key Responsibilities:
Deliver high-quality, systemically-informed adult social care practice within Southwark's practice framework.
Work directly on hospital wards, supporting patients and families through the discharge process.
Conduct complex needs assessments, Mental Capacity Assessments, and discharge planning meetings.
Collaborate with a multidisciplinary team including medical professionals, therapists, and families to ensure safe, timely discharges.
Manage complex caseloads while promoting independence, safeguarding vulnerable adults, and ensuring the best possible outcomes.
Provide mentorship and support to less experienced colleagues and students, helping to develop best practice within the team.
? Requirements:
SWE Registration
Strong experience working in hospital discharge, older adults, and/or physical disabilities
Ability to carry out complex assessments and manage challenging cases under pressure
Experience with Mental Capacity Act, Care Act, and safeguarding procedures
Proficient in collaborative working and confident in discharge planning meetings
