We are currently recruiting 2 experienced Qualified Social Workers (QSWs) to join a newly established Mental Health Hospital Discharge Team in Nottinghamshire. This is a fantastic opportunity to be part of a dynamic, fast-paced service focused on improving outcomes for individuals transitioning from hospital back into the community.

Role Overview:
Work with individuals with mental health needs within acute hospital settings and the wider community.

Undertake timely Care Act assessments to support safe and effective discharge planning.

Liaise and coordinate with multi-disciplinary teams (MDTs) including ward staff, psychiatrists, and community teams.

Promote a Home First approach, ensuring person-centred planning, safeguarding awareness, and strength-based support.

Complete Mental Capacity Assessments (MCA) and best interest decisions where required.

Essential Requirements:
Social Work England registration

Minimum 2 years' experience in a mental health setting, including working in or with acute hospitals

Solid knowledge of the Care Act, Mental Capacity Act, and discharge planning legislation

Confidence working within hospital wards and multi-agency environments

Additional Information:
Must be able to visit local hospitals and community settings in Nottinghamshire.

Hybrid working options may be considered, but on-site presence is essential for ward liaison and assessments.
