As a QSW in the LD Care Management Team, your key responsibilities will include:
- Conducting comprehensive assessments to identify the needs and strengths of individuals with learning disabilities.
- Developing and implementing person-centered care plans in collaboration with clients, families, and other professionals.
- Monitoring and reviewing care plans to ensure effective service delivery and positive outcomes.
- Providing advocacy and support to clients and their families, ensuring their voices are heard.
- Maintaining accurate and timely documentation in accordance with policy and regulatory requirements.
- Engaging in multidisciplinary team meetings and contributing to collaborative approaches to care.
The ideal candidate will possess the following essential qualifications and skills:
- A recognized social work qualification (e.g., Degree in Social Work, CQSW, DipSW).
- Registered with Social Work England.
- Proven experience in working with individuals with learning disabilities.
- Strong assessment, analytical, and communication skills.
- Ability to work collaboratively within a team and with external partners.
- Knowledge of relevant legislation and best practices in social work.
