We Are Recruiting - Join Derby's Central Community Support Team

We are seeking an experienced and motivated Qualified Social Worker to join the Central Community Support Team supporting adults across Derby City.

The team works with individuals who have a range of primary needs, including:

  • Physical disabilities

  • Frailty

  • Mental health support needs

  • Learning disabilities

  • Autism

This is a fast-paced and rewarding role within a busy area of the city. The successful candidate must be confident working independently and completing home visits within the community.


Key Responsibilities

  • Completing Care Act assessments, reviews, and person-centred support plans.

  • Working in partnership with customers, carers, and multidisciplinary professionals.

  • Undertaking safeguarding enquiries and chairing strategy meetings.

  • Applying Making Safeguarding Personal principles and proportionate risk management.

  • Using a strengths-based approach aligned with the three conversation model.

  • Considering and accessing Continuing Healthcare (CHC) funding.

  • Recording case work accurately and efficiently using Liquid Logic.


Essential Requirements

Applicants must have:

  • Recent, hands-on experience using Liquid Logic, with confidence working autonomously.

  • Strong working knowledge and legal literacy of:

    • Care Act 2014

    • Equality Act 2010

    • Mental Capacity Act 2005

  • Experience completing safeguarding enquiries and chairing strategy meetings.

  • Experience completing Care Act assessments and reviews.

  • Experience working within a strengths-based, three conversation model framework.

  • Knowledge of Continuing Healthcare (CHC) processes.

  • Ability to manage workload effectively in a busy team.

  • Social Work qualification and registration with Social Work England.

  • Ability to undertake community-based home visits across Derby City.
