We are seeking a dedicated and passionate Qualified Social Worker to join our Adult's SW - Transitions Team in Rotherham. This dynamic role involves working with adults who are transitioning between care services, ensuring they receive the support they need to lead fulfilling and independent lives. You will play a vital role in assessing needs, developing care plans, and collaborating with a variety of stakeholders to facilitate smooth transitions.

Key responsibilities include:

  • Conducting comprehensive assessments of individuals transitioning from children's services to adult services.
  • Developing and implementing person-centered care plans in collaboration with clients and their families.
  • Providing ongoing support and guidance to clients during their transition period.
  • Working closely with multidisciplinary teams, including healthcare providers and community services, to ensure holistic support.
  • Monitoring and reviewing care plans to adapt to changing needs and circumstances.

To be considered for this role, you must possess the following qualifications and skills:

  • A recognized social work qualification (e.g., BA/MA in Social Work).
  • Current registration with Social Work England.
  • Experience in adult social work, particularly in transitions or related fields.
  • Strong communication and interpersonal skills to engage effectively with clients and professionals.
  • Ability to work independently and as part of a team, demonstrating excellent organizational skills.
