Job Title: Experienced Occupational Therapist - Adults and Older People's Review Team

Eden Brown Synergy is looking for an experienced HCPC-registered Occupational Therapist to join the Adults and Older People's Review Team.

Role Overview:
You will be responsible for carrying out annual reviews and reassessments, evaluating current care packages, and recommending changes where required. You will work collaboratively with multidisciplinary teams, carers, and partners, maintaining accurate records and upholding professional standards at all times.

Key Responsibilities:

  • Conduct annual reviews and reassessments for adults and older people
  • Evaluate existing care packages and recommend modifications
  • Collaborate effectively with multidisciplinary teams and external partners
  • Support carers and service users in care planning
  • Maintain accurate and timely records, ensuring compliance with professional standards
