Eden Brown Synergy is recruiting for a Qualified Social Worker to join the Adults Review Team S117 (Aftercare Service) in partnership with Rotherham Metropolitan Borough Council (RMBC) and Rotherham, Doncaster & South Humber NHS Trust (RDaSH).

This role involves reviewing eligibility for aftercare services under S117 of the Mental Health Act 1983 and working collaboratively with health professionals to assess individual needs under the Care Act 2014.

Key Responsibilities:

  • Review and assess eligibility for Aftercare services (S117 Mental Health Act 1983).
  • Complete and update support plans for individuals with assessed needs under the Care Act 2014.
  • Work alongside health professionals in a multi-disciplinary team to deliver high-quality service.

What we are looking for:

  • Qualified Social Worker with experience in Mental Health or Adults Services.
  • Prior experience with S117 reviews and Care Act 2014 support planning is essential.
  • Ability to work collaboratively in a multidisciplinary setting.
