Monday to Friday | 9:00 AM - 5:00 PM (37 hours)
Contract until 31st August 2026
Hybrid Working - Office / Home / Client Visits

We are looking for an experienced Senior Social Worker to join the Adults with Learning Disabilities & Autism Team in Telford & Wrekin.

This role will focus on:

  • Completing Care Act Reviews
  • Maximising healthcare funding opportunities
  • Managing a social work caseload
  • Supporting assessment, support planning, and reviews using an asset-based approach
  • Contributing to weekly peer reviews and case discussions
  • Working closely with individuals, families, council teams, and partner agencies

Requirements:

  • Social Work qualification and registration
  • Enhanced DBS (Adults & Children's Barred List)
  • Full UK Driving Licence
  • Experience within Adult Social Care
