Job Title: Senior Social Worker - Early Intervention & Prevention Team
Location: Telford & Wrekin (Hybrid - Office / Home / Community)
Pay Rate: £31.97 per hour (Umbrella)
Hours: Monday to Friday, 9:00am - 5:00pm (37 hours per week)
Overview
Telford & Wrekin Council are seeking an experienced Senior Social Worker to join their Early Intervention & Prevention Team, working within a strengths-based, Community Led Support and asset-based practice framework.
This is a hybrid role combining office-based work, remote working, and direct client visits, with a strong emphasis on promoting independence, preventing escalation of need, and ensuring high-quality outcomes for adults in the community.
Key Responsibilities
- Manage a professional adult social work caseload within Early Intervention & Prevention services
- Complete Care Act 2014 assessments, support planning, and statutory reviews
- Undertake Care Act Reviews with a strong focus on maximising Continuing Healthcare (CHC) funding opportunities
- Apply Mental Capacity Act 2005 principles in assessment and decision-making
- Work in accordance with the Care Act 2014, Human Rights Act 1998, and safeguarding legislation
- Undertake proportionate risk assessments and safeguarding enquiries, ensuring timely and effective interventions
- Participate in weekly peer review meetings to support reflective case discussion and decision-making
- Work collaboratively with individuals, families, carers, and multi-agency partners to deliver integrated support
- Promote strengths-based practice and community-based solutions
- Maintain accurate, timely, and high-quality case recording in line with local authority standards
- Contribute to service development, reflective practice, and continuous professional improvement
