As a Qualified Social Worker in our team, your key responsibilities will include:
- Conducting comprehensive assessments of individuals’ needs, risks, and strengths.
- Developing and implementing person-centered care plans in collaboration with service users and their families.
- Providing ongoing support and advocacy to individuals, ensuring their voices are heard and respected.
- Working collaboratively with multi-disciplinary teams and external agencies to coordinate services and resources.
- Maintaining accurate and up-to-date records in accordance with local policies and statutory requirements.
To be considered for this role, you must possess the following essential qualifications and skills:
- A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
- Registration with Social Work England.
- Experience working with adults with learning disabilities or in a community setting.
- Strong assessment and intervention skills, with a clear understanding of relevant legislation.
- Excellent communication and interpersonal skills, with the ability to build rapport with diverse individuals.
