We are seeking a passionate and dedicated Qualified Social Worker to join our Adult's Social Work Assessment Team in Nottingham. You will work collaboratively within a supportive team to conduct assessments, develop care plans, and ensure that our service users receive the highest quality of care and support.
As a member of the Assessment Team, your key responsibilities will include:
- Conducting comprehensive assessments of adults in need of support, ensuring that their needs and preferences are fully understood.
- Developing and implementing individualized care plans in collaboration with service users, families, and other professionals.
- Providing ongoing support and monitoring to ensure the effectiveness of care plans and making adjustments as necessary.
- Working in partnership with multidisciplinary teams and external agencies to coordinate services and resources.
- Maintaining accurate and up-to-date records in compliance with statutory requirements and organizational policies.
The ideal candidate will possess the following essential qualifications and skills:
- A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
- Current registration with Social Work England.
- Demonstrable experience in adult social work, particularly in assessment and care planning.
- Strong communication and interpersonal skills, with the ability to build rapport with diverse client groups.
- Excellent organizational skills and the ability to manage a caseload effectively.
