As a Social Worker in our LD & Early Adulthood Team, you will be responsible for:
- Conducting comprehensive assessments to identify the needs and strengths of individuals with learning disabilities.
- Developing and implementing person-centered support plans in collaboration with service users and their families.
- Providing ongoing support and advocacy to empower individuals in achieving their goals and aspirations.
- Working closely with multidisciplinary teams to ensure a holistic approach to care and support.
- Maintaining accurate and timely records in accordance with statutory requirements and best practice guidelines.
The ideal candidate will possess the following qualifications and skills:
- A recognized social work qualification (BA/MA in Social Work, CQSW, DipSW, or equivalent).
- Current registration with Social Work England.
- Experience working with adults with learning disabilities and knowledge of relevant legislation.
- Strong assessment and analytical skills, with the ability to work independently and as part of a team.
- Excellent communication and interpersonal skills, with a commitment to promoting the rights and dignity of individuals.
