We are currently seeking a dedicated and experienced Qualified Social Worker to join our CHC Team in Nottingham. As a key member of the team, you will be responsible for assessing the needs of individuals seeking Continuing Healthcare (CHC) support, ensuring that they receive the appropriate care and services to meet their specific requirements.
Key responsibilities and duties include:
- Conducting comprehensive assessments of service users to determine their eligibility for CHC funding
- Developing care plans in collaboration with service users, their families, and other healthcare professionals
- Advocating for the rights and best interests of service users, ensuring they receive high-quality care and support
- Participating in multidisciplinary meetings and reviews to discuss and evaluate service user care plans
- Providing guidance and support to colleagues and stakeholders on CHC processes and regulations
Essential qualifications and skills:
- Qualified Social Worker with current registration with Social Work England
- Experience working in a similar role within a CHC setting
- Strong assessment and care planning skills
- Excellent communication and interpersonal abilities
- Ability to work effectively within a multidisciplinary team
Additional requirements:
- Ability to travel within the Nottingham area to conduct assessments and home visits
- Knowledge of relevant legislation and policies related to CHC
