Adult's SW - Assessment Team - Wokingham
We are currently seeking a dedicated and experienced Adult's Social Worker to join our Assessment Team in Wokingham. As a key member of the team, you will be responsible for conducting comprehensive assessments of adults in need of support and services to promote their well-being and independence.
Key responsibilities and duties:
- Undertake assessments of adults with complex needs to determine appropriate support and intervention
- Develop care plans in collaboration with service users, their families, and other professionals
- Provide ongoing support and review services to ensure the needs of service users are met
- Work closely with partner agencies to coordinate and deliver integrated services
- Maintain accurate and up-to-date records in accordance with statutory requirements
Essential qualifications and skills:
- Qualified Social Worker with a recognized social work qualification
- Registered with Social Work England
- Demonstrated experience in adult social work, particularly in assessments and care planning
- Strong communication and interpersonal skills
- Ability to work effectively within a multi-disciplinary team
