We are seeking an experienced and dedicated Social Worker to join the Community Adult Social Care team in North Lincolnshire. This is a hybrid role requiring a flexible approach to working between home, the office, and visiting clients in the community.

Key Responsibilities:

Manage a caseload of adults requiring long-term social care support

Undertake person-centred assessments in line with the Care Act 2014

Carry out safeguarding enquiries and contribute to safeguarding plans

Complete Mental Capacity Assessments (MCA) where appropriate

Develop support plans and work collaboratively with other professionals

Maintain accurate records and comply with statutory requirements

Requirements:

Qualified Social Worker with current Social Work England registration

Proven experience of working with adults in a statutory setting

Strong assessment and case management skills

Sound knowledge of the Care Act, Mental Capacity Act, and safeguarding procedures

Excellent communication and interpersonal skills

Ability to work independently and manage time effectively

Must be able to travel and work within North Lincolnshire as required
