We're currently seeking a skilled Reablement OT to join our supportive team in Cheshire West.

Key Responsibilities:
Conduct holistic occupational therapy assessments to support individuals in regaining independence.

Review existing care packages, ensuring they are tailored and effective in meeting client needs.

Collaborate with multidisciplinary teams to deliver high-quality reablement services.

Promote client-centred care, enabling individuals to remain in their own homes for as long as possible.

Requirements:
HCPC registered Occupational Therapist

Previous experience in reablement or a community-based setting preferred

Strong communication and assessment skills

Ability to work independently and manage a varied caseload
