We are currently seeking a passionate and dedicated Qualified Social Worker to join our team at the Cheshire East - TM - Children with Disabilities Hub. As a key member of our team, you will play a crucial role in providing support and services to children with disabilities and their families.

Key responsibilities and duties:

  • Assessing the needs of children with disabilities and developing care plans
  • Working collaboratively with families, healthcare professionals, and other agencies to ensure the best outcomes for the children
  • Providing advocacy and support to children and families to navigate the social care system
  • Participating in multidisciplinary team meetings and case conferences
  • Monitoring and reviewing the progress of children in care and making necessary adjustments to support plans

Essential qualifications and skills:

  • Qualified Social Worker with a valid registration with the Health and Care Professions Council (HCPC)
  • Demonstrated experience working with children with disabilities and their families
  • Strong assessment and case management skills
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced and challenging environment
