Qualified Social Worker - Community Active Recovery Team - Waltham Forest Council

Contract Type: Locum
Pay Rate: Competitive (DOE)

Waltham Forest Council is seeking an experienced Qualified Social Worker to join their Community Active Recovery Team. This team focuses on providing short-term interventions to support adults post-discharge and in crisis prevention.

Duties include:

Completing Care Act assessments

Developing support and reablement plans

Coordinating with health and care professionals to deliver timely interventions

Conducting safeguarding enquiries and mental capacity assessments

Supporting service users to regain independence and reduce long-term care needs

Ideal Candidate:

SWE registration

Experience in community or hospital discharge settings

Excellent knowledge of legislation including the Care Act and MCA

Able to work both independently and within a multi-disciplinary team
