As a key member of our team, you will work collaboratively with individuals, families, and other professionals to promote independence and enhance well-being.
Your key responsibilities will include:
- Conducting comprehensive assessments to identify the needs and preferences of individuals with learning disabilities.
- Developing and implementing person-centered support plans that promote independence and quality of life.
- Working closely with families and carers to provide guidance, support, and education.
- Collaborating with multi-disciplinary teams to ensure a holistic approach to care and support.
- Maintaining accurate and up-to-date records, ensuring compliance with relevant legislation and policies.
To be successful in this role, you must possess the following essential qualifications and skills:
- A recognized social work qualification (e.g., Degree in Social Work, Diploma in Social Work).
- Registration with Social Work England.
- Experience working with adults with learning disabilities or in a similar setting.
- Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
- Excellent organizational skills and the ability to manage a caseload effectively.
