The London Borough of Lambeth is seeking a dedicated and passionate Qualified Social Worker to join our Adult's Assessment Team. Key responsibilities and duties include:

  • Conducting comprehensive assessments of individuals’ needs and strengths.
  • Developing and implementing person-centered care plans in partnership with service users and their families.
  • Monitoring and reviewing care plans to ensure effectiveness and compliance with best practices.
  • Working collaboratively with health services, community organizations, and other professionals.
  • Providing guidance and support to less experienced team members and participating in team meetings and training sessions.

To be considered for this role, candidates must possess the following essential qualifications and skills:

  • A recognized Social Work qualification (e.g., BA/MA in Social Work).
  • Current registration with Social Work England.
  • Proven experience in adult social work, particularly in assessment and care planning.
  • Strong communication and interpersonal skills, with the ability to build rapport with service users and colleagues.
  • Excellent organizational skills and the ability to manage a varied caseload effectively.
