Leeds- Advance Practitioner-Locality Team

Your key responsibilities will include:
- Assessing and reviewing the needs of service users, developing and implementing care plans.
- Providing expert guidance and support to less experienced practitioners and social workers.
- Ensuring compliance with relevant legislation, policies, and procedures.
- Maintaining accurate and up-to-date records, contributing to case reviews and audits.
- Building strong relationships with external agencies, stakeholders, and community resources.
The ideal candidate will possess the following essential qualifications and skills:
- A relevant social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
- Registered with Social Work England.
- Significant experience in a social care setting, ideally within locality or community teams.
- Strong assessment, analytical, and decision-making skills.
- Excellent communication and interpersonal skills, with the ability to engage effectively with diverse populations.
