Adult's SW - Ageing Well Team - City of Wolverhampton
The City of Wolverhampton is seeking a dedicated and compassionate Qualified Social Worker to join our Ageing Well Team within the Adult's Social Work department. This role is pivotal in promoting the well-being and independence of older adults in our community. As a member of our team, you will work collaboratively with individuals, families, and other professionals to support our commitment to enhancing the quality of life for older residents.
Your key responsibilities will include:
- Conducting comprehensive assessments of individual needs and circumstances to develop tailored support plans.
- Providing direct support and guidance to older adults and their families, ensuring access to necessary services and resources.
- Collaborating with healthcare providers, community organizations, and other stakeholders to facilitate holistic care and support.
- Advocating for the rights and needs of older adults, ensuring their voices are heard and respected.
- Maintaining accurate records and documentation in compliance with local and national regulations.
The ideal candidate will possess the following essential qualifications and skills:
- A recognized Social Work qualification (e.g., BA/MA in Social Work).
- Current registration with Social Work England.
- Proven experience in adult social work, particularly with older adults.
- Strong communication and interpersonal skills, with the ability to build rapport with clients and collaborate effectively with multidisciplinary teams.
- Excellent problem-solving abilities and a compassionate approach to service delivery.
