We are seeking a dedicated and passionate Adult's Social Worker to join our Locality Team in Newcastle. This is an exciting opportunity for a qualified professional to make a meaningful impact on the lives of adults within our community. As a member of our dynamic team, you will be responsible for delivering high-quality social work services, ensuring the well-being and safety of vulnerable adults.

Your key responsibilities will include:

  • Conducting comprehensive assessments of individual needs and circumstances.
  • Developing and implementing person-centered support plans.
  • Collaborating with multi-disciplinary teams and external agencies to coordinate care and support.
  • Providing ongoing monitoring and review of care plans to ensure they meet evolving needs.
  • Advocating for service users and empowering them to access appropriate resources and services.

To be successful in this role, you will need:

  • A recognized social work qualification (e.g., BA/MA in Social Work).
  • Registration with Social Work England.
  • Experience in adult social work, particularly in community settings.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
