North Lincolnshire - Adults Social Worker - Community Team
North Lincolnshire Council is seeking a dedicated and compassionate Adults Social Worker to join our dynamic Community Team. In this role, you will play a crucial part in supporting vulnerable adults to lead fulfilling lives within their communities. You will work collaboratively with individuals, their families, and other professionals to assess needs, develop care plans, and provide ongoing support to ensure the best possible outcomes for those you serve.
Key responsibilities include:
- Conducting comprehensive assessments of individuals' needs and circumstances.
- Developing and implementing person-centred care plans in collaboration with clients and their families.
- Providing ongoing support and monitoring of care plans to ensure effectiveness and address any changes in circumstances.
- Working closely with other agencies and professionals to coordinate services and support for clients.
- Maintaining accurate records and documentation in compliance with legal and regulatory requirements.
To be considered for this position, you must possess a relevant social work qualification (Degree in Social Work or equivalent) and be registered with Social Work England. Additionally, you should have:
- A strong understanding of relevant legislation and best practices in adult social care.
- Excellent communication and interpersonal skills.
- The ability to work independently and manage a caseload effectively.
- Empathy, resilience, and a commitment to promoting the welfare of vulnerable adults.
