In this role, you will work collaboratively with individuals, families, and partner agencies to assess needs and deliver high-quality support to promote independence and well-being.

Your key responsibilities will include:

  • Conducting comprehensive assessments to identify the needs and strengths of service users.
  • Developing and implementing care plans in partnership with clients and their families.
  • Providing ongoing support and monitoring of client progress, ensuring that services are responsive to changing needs.
  • Collaborating with multidisciplinary teams and external agencies to deliver integrated services.
  • Maintaining accurate and up-to-date case records and documentation in accordance with legal and organizational standards.

To be successful in this role, you must possess the following qualifications and skills:

  • A relevant social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Current registration with Social Work England.
  • Experience working with adults in a social work setting, preferably within a locality team.
  • Strong assessment and analytical skills, with the ability to formulate effective care plans.
  • Excellent communication and interpersonal skills, with a compassionate approach to service delivery.
