We are currently seeking a passionate and dedicated Qualified Social Worker (QSW) to join our Adult Social Care team in Hackney. As a key member of our team, you will be responsible for providing high-quality support and intervention services to vulnerable adults in the community.
Key responsibilities and duties include:
- Conducting assessments to determine the needs of service users and develop care plans accordingly.
- Working collaboratively with multi-disciplinary teams to ensure holistic support for service users.
- Providing advocacy and support to empower service users in making informed decisions about their care.
- Monitoring and reviewing care plans regularly to ensure effectiveness and make necessary adjustments.
- Participating in safeguarding procedures and taking appropriate actions to protect vulnerable adults.
- Qualified Social Worker (QSW) with current registration with the Health and Care Professions Council (HCPC).
- Experience working in Adult Social Care, preferably within a similar setting.
- Strong communication and interpersonal skills, with the ability to build rapport with service users and their families.
- Excellent assessment and decision-making skills to provide effective support and intervention.
- Ability to work autonomously and as part of a team in a fast-paced environment.
