Assessment Social Worker - South Tyneside

We are seeking a dedicated and experienced Assessment Social Worker to join our team in South Tyneside. In this role, you will be responsible for conducting assessments and providing support to individuals and families in need within the community.

Key responsibilities and duties include:

  • Conducting comprehensive assessments to determine the needs of service users
  • Developing and implementing care plans in collaboration with service users and their support networks
  • Providing ongoing support and guidance to service users to promote their well-being and independence
  • Working closely with other professionals and agencies to coordinate services and resources for service users
  • Participating in case conferences and reviews to ensure the effectiveness of interventions

The ideal candidate for this position will possess the following qualifications and skills:

  • Qualified Social Worker with a valid registration with Social Work England
  • Demonstrated experience in conducting assessments and developing care plans
  • Strong communication and interpersonal skills to engage effectively with service users and stakeholders
  • Ability to work independently and as part of a multidisciplinary team
  • Knowledge of relevant legislation and policies related to social work practice
