Are you passionate about promoting independence, recovery, and strengths-based practice? Southwark Council's Intermediate Care Service (ICS) is seeking a committed and skilled Social Worker to join the Reablement Team. This is a fantastic opportunity to work in a multi-disciplinary setting alongside health professionals, where you'll make a meaningful impact on peoples lives during times of crisis or transition.

Key Responsibilities:
Undertake Care Act assessments and develop person-centred care and support plans

Complete reviews, Mental Capacity Assessments, and chair Best Interests meetings

Set reablement and rehabilitation goals with service users

Provide timely signposting, advice, and information to individuals and families

Work with individuals facing complex health and social care needs, including carers

Carry out initial safeguarding fact-finding, assess and manage risks to vulnerable adults

What Were Looking For:
Qualified Social Worker with Social Work England registration

Experience working within Adult Social Care

Strong understanding of the Care Act, Mental Capacity Act, and safeguarding procedures

Ability to work autonomously and as part of a multi-disciplinary team
