We are currently seeking a dedicated and experienced Adult's Social Worker to join our Mental Health Hospital Team in Leicestershire. As a key member of our team, you will be responsible for providing high-quality social work services to adults with mental health needs, ensuring their well-being and promoting their independence.
Key responsibilities and duties include:
- Conducting assessments to identify the needs of service users and develop care plans
- Working collaboratively with multidisciplinary teams to coordinate and deliver integrated care
- Providing support and advocacy for service users to access appropriate services and resources
- Participating in safeguarding procedures and risk assessments to ensure the safety of vulnerable adults
- Maintaining accurate and up-to-date records of interventions and outcomes
The ideal candidate will possess the following essential qualifications and skills:
- Qualified Social Worker with a valid HCPC registration
- Experience working with adults with mental health issues in a hospital or community setting
- Strong assessment and care planning skills
- Excellent communication and interpersonal skills
- Ability to work effectively under pressure and manage a complex caseload
In addition to the above requirements, candidates with experience in mental health legislation and knowledge of relevant policies and procedures will be highly desirable.
