Duties and Responsibilities:
* Comprehensive Assessments: Conduct in-depth children and family assessments to accurately identify an individuals strengths, needs, and risks.
* Care Planning: Develop, implement, and review tailored care and support plans to meet physical, educational, emotional, and medical needs.
* Safeguarding: Manage child protection and safeguarding cases, ensuring the safety of vulnerable individuals and their non-disabled siblings.
* Multidisciplinary Collaboration.
* Legal Proceedings: Manage cases in the legal arena, including compiling reports and care plans for public or private law processes and Looked After Child (LAC) reviews.

COMMENTS:
This role involves working with children and young people aged 0-18 who have complex health needs and disabilities. The successful candidate will manage a varied caseload, including Child in Need (CIN), Child Protection (CP), and Children in Care (CIC), while also working closely in partnership with the Short Breaks Team.
